Green fabric covered booklet with black text reading "CANADIAN ARMY (A.F.) OFFICER'S RECORD OF SERVICE". The first page of the booklet has instructions, the second has his personal information, the third has the list of his promotions, the fourth is a list of courses he took, the fifth is his service, sixth is medical classification, seventh is hospitalization, and the eighth page has protective inoculations and vaccinations.

Use

This booklet was central to recording information of officers in the Canadian Army during the Second World War. It contains all his personal information, his progress in the military, his health, etc.
