This document is addressed to 井上 半三郎 殿 Ino'ue Hanzaburō dono (殿, a more respectful way to address than just 氏 shi).
This appears to be a memo of some sort, the title heading section reading:
御幸(?)公之覚 Go kō(?) kō no oboe (memo)
私？御幸(?) I ?
? ? ?
左之通 hidari (or sa, left) no(=of) tori
Item one: 一 月？二十四日 (some month) 24th day 病中 byōchū (in bed).
This section gives two more items to the memo, whose details will require more time to figure out.
The last section gives the date and the name of the one who wrote this memo:
右之通御？之以上 (As noted to the right)
文政九年 Bunsei 9th year, 1826, 六月 sixth month
当年四十七歳？？？ this year, at the age of 47
Signed: 中嶋 五？助 Nakajima Go?suke, followed by his（花押 kaō）and seal.
