Disc is housed inside a black case with a white cover and crossed-out barcode. Along the side of the case is the title, and inside, there is a blank disc quality check form, a remnant from the DVD's time at the library. A note inside the case indicates that the disc was last resurfaced and previewed on October 5, 2011.
