Certificate- This certifies that I, the undersigned, hereby agree that the bearer (and immediate family), being an owner of the COPY OF HEALTH is entitled to and shall receive free and prompt advice in answer to any query of medical nature, either by mail or in person at my office for a period of to years.
This further certifies that the bearer will receive free physical examination and medical consultation. In weeking advice, carefully fill out the symptom blanks and give further information possible. Also give full name and Post Office address, plainly written. Always enclose TWO CENT STAMP for reply.

Any prescription you desire will be sent to you promptly, so you can have it filled by your local druggist without unnaccaryly delay. Should your druggist at any time, for any reason, fail to supply prescriptions or treatments called for, write me at once, explaining clearly the difficulty, and you will receive a written and signed prescription without any cost to you, and advice as how to obtain what you need.
It is required in every case that you write clearly your full name and address, describing your symptoms and stating what you want, and also that you enclose a TWO CENT STAMP for reply, otherwise the correspondent will not receive attention.
