A green Birth Certificate card for David Lee Tyndall. Printed along the top is a "Certificate of Birth." Then "Registration Division of the Township of Whitby, County of Ontario." "This is to Certify that the following particulars are from a registration filed in the said registration division: Name David Lee Tyndall, Sex Male, Date of birth November 21st, 1948, Place of Borth Lor 26, Con. 3, Full Name of Father, Carman Squire Tyndall, Maiden Name of Mother, Kathleen Joan Lee, Date of Registration December 10th, 1948, This is to certify further that the said registration was so filed during the preceding three months before the issue hereof, given under my hand this 10th day of December 1948, Murray Robinson Divison Registrar." Written along the bottom next to a pointing hand is "A division registrar, upon application therefor, and on payment of a fee of twenty-five cents shall give a certificate in the prescribed form as any registration filed with him during the preceding three months, but shall not give any certificate other than such a registration other than within the said three months can be issued only by the registrar-general, parliament building, Toronto 15000-March, 1940 (P4912)."
