The black-and-white portrait shows the bust of a white, adult man with dark, short hair parted in the middle, large ears, and a neutral expression looking away from the camera towards the left of the print. The man is dressed in a buttoned black suit jacket or coat, made from a thick, textured material, a white shirt with a flared collar, and a polka-dot tie, which has a pin in the shape of two overlapping hearts. The background behind the man is a soft gray color. There is fading along the bottom edge of the image, as well as two areas of scratches around the collar of the man's suit jacket. On the reverse of the image there is adhesive residue along the edges and concentrated in the bottom right corner. There is a centered stamp located slightly above the adhesive residue along the bottom edge, which reads "PHOTOGRAPH BY / JUDSON SMITH STUDIO / FREDERICKSBURG, VA." in black ink. There are also three hand-written inscriptions. The first of these is the largest, occupying the approximate middle-third of the print. It is written in print and blue ink and reads "CLARANCE RANDOLPH HOWARD / ITS "A" HE WAS NAMED AFTER HIS / MOTHER CLARA / BORN DEC. 4, 1871 / DIED NOV. 10, 1934." Between the first and second lines of the inscription is a diagonal arrow pointing from the letter A in quotation marks in the second line to the underlined letter A in the first line. The second print inscription is also written in blue ink and is located to the left of the word "PHOTOGRAPH" of the centered stamp. It reads "Copy." The third print inscription is in black ink and located to the right of the centered stamp. It reads "1971."
