CHLOROFORM 5 MINIMS PER OUNCE. Prepared only by THE PISO COMPANY, WARREN, PA. After a Prescription of DR. M. F. TALBOTT."; back panel has embossed in glass statement "THE PISO COMPANY." Right side panel is embossed with "HAZELTINE & CO."; left side panel is embossed with "TRADE - PISO'S - MARK."
