Standard fill-in-the-blanks license form. Top has "DENTISTRY LICENSE" surrounded by scroll work. In middle is a cartouche showing dentist and patient on one side, books and other symbols of learning on other side. Partial state seal in middle. Shelby County license for John E. Chadwich dated 1904. Lower left corner has a stick-on Shelby County seal of blue paper. At bottom of license in small print: "Wm. B. Condition: Black wood frame is marred around the edges. License is on high acid paper, very similar to newsprint. Edges of license are brittle. Everything was taken apart and cleaned 07/20/1984 and acid-free tissue paper inserted between the license and a new cardboard backing.Create Date
