Ink faded, difficult to read, 1-1/4" x 7/8" hole in center of document, 1-5/8" x 1" section missing left edge, 1-1/8" x 5/8" damaged section top edge, right edge curled under near top, discoloring or staining on top edge, lower right corner and generally throughout document; lower left corner area for seal has series of pen strokes 3 of which cut the document. Backside 1 horizontal and at least 2 vertical lines or creases from where document was once folded.
