A & B: Bronze and silver colored medal attached to red ribbon. Overall shape is bronze with cutout relief of two cannons crossed over with Maltese cross over laurel wreath. At top are two crossed swords. At top says "15" (in silver) and "YEARS" underneath. Then says "V.C.A." (in silver) over Eagle with head facing left and wings spread open with "1790" across breast plate in silver. And "LONG AND FAITHFUL SERVICE" across banner on bottom (in silver) and "N.Y." in brass on Maltese cross. Red ribbon has brass colored buckle at end with two prongs.
C: Light blue paper covered jewelry (storage) box with dark blue raised print on top center with logo and reads "DIEGES & CLUST/ MFG. SPECIALTY JEWELERS/ BOSTON-NEW YORK/ PITTSBURGH/ CHICAGO. Paper sticker of rectangular shape with red border and black handwriting that reads "15 years/ 5". Interior lid is ivory cardboard. Bottom has interior raised lip that lid fits over and is lined with padded ivory (with small diamond shaped texture) satin. At one end is an elastic across with and at other is a raised section with slit for medal to fit into.
