Order of Commitment to St. Mary's Hospital, Hoboken, N.J. No. 263, Overseer of the Poor, Oct. 13, 1909. Printed form, press numbered, edge perforations; 3-3/4" high x 6-3/16" wide; holographic ink entries. Bearer was Adolph Frank of 211 Park Ave. Recommended by Dr. Muncie(?) Signed by Harry L. Barck. Hospital charges would be paid by the City of Hoboken. Case was cancelled as the line "When Discharged" is blank. Below that line is "This voucher to be returned with bill to Overseer of the Poor." This voucher was in a 1910 group with an envelope labeled "Sick & Indigent Poor" "Canceled Cases." See archives 2010.010.0068 for that group.
