List of Person who Failed to: Submit Questionaires. Local Board No. 1, Hoboken, January 11, 1919. Four leaves with typewritten entries and holographic annotations, 8" x 10-1/2" high. P.M.G.O. Form 1013. Digital images only. Tiffs media archive. Names are transcribed alphabetically without remarks or other date in notes field. The initial registration date was September 12, 1918 with data recorded from later dates for each man listed who did submit forms. Two of the four sheets are signed in facsimile by Edward J. Sharkey of the Local Board. P.M.G.O. [Provost Marshall General Office] form also includes in the title and body text: Report for Physical Examination. It is either crossed out in ink or 'x'ed out with typewriter on all leaves. Local Board No. 1 was the "draft board" as well as performing related functions under the Selective Service Act in World War I.
