Description
A prescription blank under the National Prohibition Act, 12 December 1923. The form from the Treasury Department is for the prescription of an alcoholic beverage considered a medicine to a Los Angeles resident and has that person's name and address, that of the pharmacy and that of the doctor. There is a space at the lower left for the pharmacist, as well. Aside from some browning, there is little wear and the condition is good.
