An order form from "Art Needlework and Fancy Wear for Women and Children," a catalog for the Fall and Winter of 1928-29 issued by Frederick Herrschner Inc. The form has the company name and address at the top center, a box for the customer address and statements on terms of payment and a guarantee of quality below this and rows and columns for placing the order. A forwarding address box is at the lower left with tracking information at the lower right. On the reverse is an area for more merchandise ordering and information at the bottom on remittances, C. O. D. ordering and service. The form was tipped into a page of the catalog, which is item .1. There are a few tears on the edges; creasing; dogeared corners and some discoloration to the artifact, which is in fair condition.
