On the bottom of the last line is, "PLEASE WRITE NAME AND ADDRESS PLAINLY." On the right side is the name and address of the company. Writing on the top of the back reads, "WE MAKE QUICK SHIPMENTS / YOUR ORDER SHIPPED SAME DAY / IT IS RECEIVED." A warning on the bottom reminds people to have all information written plainly on the order form before sealing the envelope. The stationery is part of the catalog which is item .1, and an order blank .2. With no visible signs of wear, the object is in good condition.
