An order form from an ESCO school suppplies catalog, issued by The Educational Supply Co., 1927. At the top of the front is the company name and address, under which are several sections for shipping; approval and billing; payment type; shipping preference; and the beginning of the listing for products ordered. These columns are for quantity; catalog number; name or description of the article; each price; and extended total. On the reverse is more room for listing products ordered. Reminders to check the form carefully for mistakes and omissions and about related products are at the bottom of both sides. Items .1 and .3 are the catalog and an envelope for this form. With some fading of the ink; acid burns from catalog pages; and many creases, the artifact is in fair condition.
