An order blank for "Embalmers, Undertakers and Cemetery Supplies: Catalog 6," issued by the Frigid Fluid Company, ca. 1910. The sheet has the name and address of the manufacturer at the top left of the front. Also at the top are blank lines for the customer's name and mailing address. At the center and bottom are many blank lines for order placement including quantity, catalog number, description, and price. On the back is a special offer for freight delivery when a case or more of embalming or arterial fluids are ordered. All of the printing is in black ink. The artifact was found within the catalog which is item .1. With only light discoloration and faint acid burns, the object is in good condition.
