An order blank issued by the Geo. H. Mellen Company for their catalog, 1927. Across the top underlined by a double line is the company name and address flanked by rectanglges, one blank, the other with the year of issue. Below this and to the left is a reminder of payment and to the right blank lines within a square meant for company use only. The lower portion has blank lines for customer information including the date, customer name and address, and amount enclosed. At the center of the form are the words, "SAY IT WITH FLOWERS." At the bottom are 16 blank lines for the order with spaces for quantity, names of plants, bulbs, and seeds, and the price of each. The form was found inserted within the catalog which is .1 along with the envelope .3. There are two folds down the center of the item which has acid stains on the top two corners, and the bottom right, but is in good condition overall.Lexicon
