GRADE REPORT, THOMAS W. TEMPLE II, LA PUENTE SCHOOL, 1915-16
A report card (a) with envelope (b) for Thomas W. Temple II from La Puente School for the academic year 1915-16. Printed on a form, one side of the report gives the name of the school; the grade level; student's name; school year; list of studies and columns for months; sections for average, number in class, and rank; and signature of teacher. On the reverse are lines for the signature of the parent and guardian for each month of the school year and the signature of the principal to promote the student to the next grade. The card is only partially filled out, with grades for the first through third and eighth and ninth months and signatures of Laura (first and third) and Walter Temple (second, fourth and fifth months.) The card is signed by teacher Carrie Broaded, but no signature was given for promotion. In fair condition, the card has a large crease in the upper right corner and smaller ones on the lower right; bent corners; and discoloration. The envelope, of plain stationery, is in poor condition with the top and right edges torn open and most of the left edge open. There are also creases; large areas of staining; and dark discoloration.
