An order blank for the "Catalogue of Fine Memorial Goods," issued by H. F. Wendell & Co., 1918. On the top of the form is, "ORDER BLANK / Send Order on this Sheet." Directions for clear writing are below as well as blank lines for order amount and quantity, style and verse numbers, and card color. At the center are blank lines for the name and birth and death dates of the deceased. Three blank squares are provided for the person's age in years, months, and days. Blank lines for the customer's name and address are below. Across the bottom is the company name and address. The form is part of the catalog which item .1, memorial card .2, and mailing envelope .4.
