An order blank issued by Loft Inc., 1926. On the top is, "ORDER BLANK," and below this is a note instructing customers to make orders on company order forms only. Also below is the name and address of the company, as well as blank lines for the customer's address and the amount of money enclosed. On the bottom are lines for the order placement, some of which are also on the back. On the bottom of the back are lines for the names and addresses of other people which would be interested in receiving the catalog. An advertisement for a kiddies' combination package is below this. The form is part of the catalog which is .1; a price list .2; and an envelope .4. With only one fold down the center, the artifact is in good condition.
