NATIONAL PROHIBITION ACT PRESCRIPTION BLANK, 1928
Certificate: A prescription blank for alcohol under the National Prohibition Act, 2 June 1928. The government certificate with a border of stylized floral and lace pattern design similar to that of paper currency has the Internal Revenue title at the top followed by the title of the document; book number; blank number; permit number; prescription information; patient's name (Lena Barnhart); cancellation date; and address and signature of the doctor (William Roane). The prescription, once folded in half, was for whiskey to be administered in eight ounce draughts before meals. Purchased from Dawson's Book Shop. The certificate is in good condition with a fold down the center and some discoloration and small creases. Much written information on the document.
