An order blank for "Modern Plumbing and Heating," issued by Sears, Roebuck and Company, 1929. The form has the name of the company across the top with many blank lines below for the customer's mailing information. At the center are blank lines for ordering information including the number of goods ordered, name of the product, size, and price. At the bottom is a section for complete plumbing system orders. At the top center of the back is, "Special / Time Payment / Order Blank." At the left and right of this are notes on payments. At the center are blank lines for company use, plumbing system orders, and for information about the building to which the products were to be delivered. Across the bottom is the name of the company and all of the printing on the artifact is in black ink. The sheet was found within the catalog which is item .1 along with two envelopes which are items .3-.4. With only a fold across the center and a crease, the object is in good condition.
