An order blank issued by the J. W. Miller Company, 1926. The yellow sheet has the company name and address on the top left corner. On the right is a square with the words, "DO NOT WRITE IN THIS SPACE." Below this are blank lines for the date, customer's mailing address, and shipping type. On the bottom are many lines for the the quantity, descrpition, and price of the items ordered. Small type across the bottom instructs people to write the name of friends on the back so they can receive the catalog. The form is part of the catalog which is item .1, and an envelope which is .3. With only light acid burns on the top corners, the artifact is in good condition.
