The form has the company name and address on the top center with instructions for payment and a guarantee at either side. Below are blank lines for the date, customer name and address, and optional shipping address. Many blank spaces for order placement are at the center, and blank lines for address information are across the bottom with the words "If You Have Moved," on the top. The form was inserted into the catalog, which is item .1, along with a return envelope, .3. With light creases on the left side; a fold across the middle; and light acid burns across the bottom, the item is in good condition.
