At the top left is addressing information for the buyer, while next to that at right is the corporation name and information on how to fill out the form. Below this are the rows and columns for writing item information, though the far left column has instructions to leave it blank, as this was the check column for verifying order shipments by the corporation. More room is on the reverse for item ordering with statements about their wholesale sales and suggestions on using a rubber stamp for the addressee.
