An order form from "The Park Nurseries," a nursery supply catalog issued by Holm & Olson, Inc., 1927. The combination form and envelope has a gatefold, one side of which forms the top of the form with the company name and address, under which are lines for the purchaser's information including name, address and shipping information. Below are columns and rows for ordering by quantity, variety name, size, each price and total. At the bottom are lines for the names of others who might benefit from information from the company. The outer sides of the gatefolds include information on customer satisfaction and how the company can help make the most of someone's yard. On the reverse is the envelope divided into halves, with the top half being the front and having the company name and address for addressee and the bottom half under a fold line warning the sender to write their name and address on the blank before sealing. Above the top portion of the envelope is a gummed seal on the order blank side. Other than slight fading, there is no discernible wear or damage to the artifact and it is in good condition.Lexicon
