Three of the four sides remain, each with the title as appears on the front cover, but two sides having the company name and address. On the reverse of the front face are directions and an offer for new sets of questionaire cards, as well as the company name and address. The artifact is in poor condition with one side missing; notable abrasions; rubbing; brittleness; creases; soiling; stains; tears; and wear throughout.
