Renewal form found in National Sportsman Magazine for November, 1928. The form has lettering in red ink and has a decorative border across the top surrounding the words, "Your Subscription / Expires/ With this Issue." Below this across the center it reads, "Renewal Order." The lower portion has a blank line for the date, the name and address of the magazine, and blank lines for the customer's name and mailing address. A border of squares lines the bottom of the paper. With light creases on the right side, a light stain on the top right corner, and a hole on the bottom right corner, the item is in overall good condition.
