Description and Notes: This two story house has elements of Greek Revival and Queen Anne styles. The Greek Revival style is evident in the cornice returns. But the Queen Anne style is present in the massing of the house and the wrap around porch.
The main portion of the house has a hipped roof with two stories. There is a central door that is bordered by a picture window and three windows clustered together. The second story windows are evenly spaced on this portion.
The other part of the house has a number of different roof lines with cornice returns and number of windows. There is a single story porch supported by round columns that is across the front facade of this portion and around the side.
The house was razed in August of 1972 for Mentor Park Hospital.
