Driver's original record and statement from Lockie-Lee Dairy. It was detached "return with remittance" stub. Dimensions: 7" x 2 and 1/2" with ribbing where detached. Some hand writing, some preprinting, and some typing on yellowed card stock quality paper. Lockie- Lee Dairy company of Painesville billed Willard Blackington of Route 44 in Chardon, Ohio 6-M the amount of $20.28 was discounted $.98 and Mr. Blackington paid $19.30 for a delivery.
