State of California Affidavit for Correction of a Record form filled out by Mother and LAG while Mother was living with LAG in Mt Lebanon PA, Sept 10, 1945. GG Lancaster was serving in WW2 in Hawii in this period. LAG will shortly leave for China. The form requested Mother's name at time of birth to be changed from Lois to Elizabeth Carolyn.
