Cherry wood box with metal closure lined in green velvet and satin holding a pewter medal strung from a blue ribbon. The medal is engraved on one side with laurel leaves surrounding the Philadelphia College of Pharmacy and Science's sigil, and on the other with the inscription: "Philadelphia College of Pharmacy and Science / Philip P. Gerbino / PharmD / February 2, 1995 / Office of the President".
