Employee's Reciept for the registation as an Insured Person under the Unemployment Insurance Act. It contains an individual employee number. Receipt belonged to E.M. Crotty and was issued by Thoburn Woollen Mills, and signed by W.A. Jamieson. The reverse side contains a chart for record of employment. It is not filled out.

EMPLOYEE'S RECEIPT
You are now registered as an Insured Person under the Unemployment Insurance Act. Your employer has your Insurance Book in his possession and it bears the same number as appears on this receipt. THIS NUMBER HAS BEEN PERMANENTLY ASSIGNED TO YOU and must be quoted in all transactions with the Unemployment Insurance Commission. If employment terminates, your book will be returned to you with contributions recorded up to date. In order to protect your rights, you must then deposit your book with the nearest Employment and Claims Office of the Unem-ployment Insurance Commission without delay. During employment, you have the right to inspect your book to insure that contributions are being properly made on your behalf.
Do not tear, soil, or lose your book when it comes into your possession as it contains a record of your contributions and any benefits to which you may be entitled will be calculated on this record.
