All the corners have been bent. The paper has been folded twice lengthwise and has torn in places along those folds. Many small brown smudges on the sides and bottom third of the paper near the fold line. Small holes near top and left corner like it was pinned to a board. There are some numbers written on the back in pencil and one word written on the front, but the rest of the paper was either printed on or written on in black ink. The writing seems to be numbers and months and the printed words are :
