2 nearly identical order forms from Collie Mills filled to Mighty Star Lid for varous orders of fabrics. The first on Nov 3rd orders 200yrs of white 13. The second orders: 2yrs white, 2yrds harvest brown, 2 yards gold, 2 yards gray mix, 2 yards hot pink, 2 yrds hot pink, 4yrds hot pink, 2yrds royal, 2 yrds royal.
The pages are as follows, the first is a white, 8.5 by 11" graph page with; fabrics ordered, shipped date, oredered date, quantity, etc. the second is a customer confirmation form from collie mills, with the same infor on the page as well as prices and the address of the recieving party. The collie logo is watermarked in teh center of the white 6 by 11"page. paper is white and printed on in black ink.
the 3rd is a Collie Mills order information page, 8.5" square listing the same information and the customer confirmation form.
order A, placed on Nov 3rd has an additional page, it is another customer confirmation form, same information is filled out as the privious form, only change is this one is signed and dated in the bottom in blue ink.
