Grant, M.D., F.R.C.S., Ed., President Dr. J.G. Giles, M.D., 1st Vice-President J.W. Pickup, M.D., 2nd Vice-President H. Hill, M.R.C.S., Eng, Treasurer J.P. Lynn, M.D., Secretary MEDICINE Ordinary visits from 7 am to 9 pm $1.00 to 4.00 Night visits from 9 pm to 7 am $2.00 to 5.00 Ordinary Consultations with other Physicians $2.00 to 5.00 Mileage from Physician's residence, first mile $2.00 Mileage from Physician's residence, each subsequent mile $0.50 to 1.00 Consultation by letter $2.00 to 5.00 Advice at Office $1.00 to 5.00 Vaccination $0.50 to 1.00 Detention per hour after first hour $1.00 to 4.00 Certificate of Lunacy or Sanity, or enquiry regarding the same $5.00 to 10.00 Examination in cases of Life Insurance not less than $5.00 Medical Certificate of any kind $5.00 For attendance in Cholera, Smallpox, et al, always double.
