Description

Form, Vehicle Collision report form used and published by Canadian National to record collisions. The form itself is meant to be given to the company's solicitor after a collision. It is blank however it has the areas to record: statement of name, occupation, length of service, date of accident, time, location, train and direction, engine number, cars and tons, crew engineer, fireman, brakeman, conductor, baggageman, speed of train before brakes applied, nature of accident, apparent cause of accident, name of, age, address, extent of injuries, owner, driver, other occupants, how were killed or injured finally cared for, kind of vehicle, car license, driver's license, direction vehicle was moving, estimated speed of, was train or vehicle at point of contact first, if vehicle struck train exactly where were first marks on engine or equipment, if train struck vehicle exactly where was vehicle struck, where did vehicle come to stop after accident, describe nature of damage to vehicle, when train stopped where was engine with respect to crossing, extent of damage to train equipment, were brakes applied before or after impact, were brakes applied in emergency by whom and where, no of cars in train with breaks in service, were brakes operating efficiently, did you hear crossing whistle or horn signals, where was it sounded, did you hear engine bell air whistle or foot gong sounding while approaching and passing over crossing, did you hear the engine bell ringing after accident, if engine bell was ringing who stopped it and when, where were you actually located when the accident occurred, how far was the vehicle from the crossing when you first saw it, how far were you from the crossing when you first saw the vehicle, what actions did you take to avoid the accident, weather conditions, condition of crossing and roadway, length of skid marks if any, if crossing lighted were street lights burning, if at night was your headlight efficiently burning full or dimmer, if protected by crossing flagman where was he located, did crossing flagman signal vehicle to stop, if crossing protected by gates wigwag or bell was such appliance working properly, was such appliance tested after accident and by whom, was it found to be in proper working order, were there any obstructions blocking view of occupants of vehicle, was vehicle's radio playing, were vehicle's headlights burning at the time of accident, if accident occurred in backup movement what lights were displayed on leading end approaching crossing and where were they located, was there a man on the leading car and where was he located, what did he do to warn highway users of the approach of the train, did you have any backup air hose and rear end whistle control, who was in charge of the same, did you have any conversation with occupants of vehicle and what did they have to say, witnesses to the accident, names, addresses, if unable to secure names of witnesses give license number of any vehicle in vicinity, how long was train held at crossing account of accident, was train properly protected according to rule 99 while so held, give whatever information in your possession not already covered by answers given above, date, signature, title, statement taken and signature witnessed by. Context

Original document used by Canadian National to record vehicle collisions. Canadian National was created following the end of the First World War, by combining five lines which were financially troubled. It soon became the largest railway in the country. Canadian National Railways was founded in 1919 as a combination of several railways that had become bankrupt and were being ran by the Canadian Government Railways, its primary service was freight but also operated passenger service until 1978 until it was absorbed by VIA rail. The Canadian National Railways was created by the Government of Canada as a means to fund the failing railway corporations including: Intercolonial Railway of Canada, National Transcontinental Railway, Prince Edward Island Railway, and Hudson's Bay Railway.
