The wreck occurred on September 13, 1906 in Azilda, Ontario due to defective brakes causing two trains to have a head on collision. A total of twelve people were killed during the wreck on one single car. The wreck occurred on the Canadian Pacific Railways line, the two trains that collided were the Imperial, and the Harvester, a westbound train originating from Toronto, Ontario.
