Small piece of blue paper printed in black ink and filled in with black ink. Along the left side is a decorative border and two lines along the top and bottom border the form. The form reads "Bought of Field, Cross & Hodges, One set of Vermont Marble Grave Stones, the Head Stone to be 5 feet long, and of proportionate width. The following inscription to be engraved thereon, to wit: Eliphalet Marthers Died, O.G. Molding/name -- in ark. With suitable Foot Stones, to delivered at Fond du Lac by the 15th day of June next, unless unforeseen causes shall prevent, and then within a reasonable time thereafter. For which I agree to pay the sum of Twenty Five Dollars on delivery. Dated, Manchester Feb 20th 1856 O.D. Fowler." The back also contained writing in black ink that reads "O.D. Fowler of Manchester Bill 25.00, July 7th 1856, Re'd on the within in full $25 Dollars, H.C. Hodges, By Bidwell."Collection
