Half sheet of blue paper with a printed form and filled in with thick black ink. The form has a label in the top left corner in a decorative square box that reads "Plain and Ornamental Job Printing, Ward's Hall, Main Street, up stairs, (Entrance on Second St.)." The form has a header between two pointing hands that reads "Blanks of all kinds kept constantly on hand." The actual form is a receipt from Oct 27, 1856 from O.D. Fowler to Beeson's Printing Office, Dr. for $7.50 of which Fowler paid.
