The plaque has the Purdue University seal (1968) on the left. On the right is: "To Don Dailey For Outstanding Professional Achievement In Industrial Design February 19, 1994". The base is made of dark-colored wood. Directly above that is a plastic, transparent block where four tall, metal plates that hold a wooden ball in place at the very top. The wooden base is one piece and the rest is another piece. Both seem to have been attached at one point but have since separated.Collection
