There is a green bar from the top left side running down vertically with "Speak Up Jaycee" in white text. There is a sponsor logo beneath it (Success Motivation Institute Inc). The Jaycees insignia is imprinted behind the text in green on bottom right side. Award is dated June 6, 1970 and signed by the President.
