Six pages of documents stapled together containing various sizes and styles from lined steno and legal pad to blank sheets containing a list of names and numbers. Some pages have headings, others do not. I cannot tell if the names are of patients, staff, visitors, or volunteers so I have tagged them as restricted. Dates are intertwined with the data and cover a period from 1969 to 1973. The information is handwritten in both pencil and ink. Due to the volume of records, I have only recorded some of each page. This collection is filed in the folder titled "Law Suit".Context
A. Eight names in alphabetical order from Don Davison to Donald Scifers. Dates run from June 6 - Present to 10/70 to 1/73. Down the left hand side is listed: 'YW2', 'YE2', and 'YW1'. at the bottom left corner is a short list of "Little boys" names.
B. Top heading "Ind." underlined. A list of 41 names and numbers: #1 being Fred Drummond 12 and #41 Larry Drummond 21. Other numbers range from 2 to 412. In the left hand column shows ' Jan 1969".
C. The page seems to have had 3" cut off. A sideways note reads " Documented Nursing Notes". There are two main names. 'Leroy Reynolds - Conception Seminary' and 3 dates is at the top. The lower name is D. Sifers accompanied by a list of women's names, titles (case worker, attendant, tracker) dates and events. There are no dates.
D. A full page with only the name Floyd Edwards - Conception Seminary - Documented in Nursing Notes, followed by a section of dates and names.
E. Title at the top "Home Visits", followed by one name, 'Bill Reynolds - Documented in Nursing' then six women's names, title and dates.
F. A white unlined small sheet of paper with information written on both sides. It is a list of churches, individuals, businesses and organizations including Conception Seminaries and McCarthy Baptist; Jaycee Wives, Fox Theater, Coca Cola, State Hosp Aux., St. Joseph Museum, K-Mart and 10 Softball teams. This could be a list of donors. There are no dates.Lexicon
