A letter on University of Missouri letterhead, from Daniel Harris, Director, Hospital Impact Study to State Hospital Employee, no dateContext
Dear State Hospital Employee:
Last year, a member of the Missouri Legislature requested the Mental Health Commission to investigate the potential effect of closing one of the state's mental hospitals. The Department of Mental Health has contracted with the Program in Health Services Management at the University of Missouri to carry out this investigation.
An Employee survey is a very important part of this investigation. You have been scientifically selected to participate in this survey. By participation, you have the opportunity to influence the Mental Health Commission and State Legislature when they discuss the possibility of a hospital closure. You become part of the decision making process.
The survey will be administered at the hospital soon b members of the University research team. You will receive information concerning where and when the survey will be conducted as soon as these details are worked out with your hospital's superintendent. The survey asks questions about your job at the hospital, your community, and how you think the closing of this hospital would affect you. It should take about 30 minutes for you to complete the survey.
At the time of the survey, the research team members will distribute the survey form to you and other selected hospital employees, instruct all of you briefly in how to fill it out, answer question's you may have concerning the survey, and then collect the completed form. The research team members will then take the surveys back to Columbia for analysis.
All survey forms are anonymous; That is, no one will be able to know who answered a particular form and no one will be able to match your answers to you personally. You will be asked not to put your name anywhere on the survey. Furthermore, no one outside the University research team will be permitted to read any of the survey forms.
I would like to thank you in advance for your cooperation. Your answers to the survey questions will become part of the information used to decide if it is feasible to close a state hospital. No decision has been made as yet. If you feel any questions or concerns regarding this survey, please feel free to contact either me at the University or Dr. Whitmore, Deputy Director for Planning and Evaluation, at the Department of Mental Health in Jefferson City. Once again, thank you for your cooperation.
Sincerely, Daniel M. Harris, Ph.D.Lexicon
