Date, case # (for first admission, re-admission, transferred in ), name, staff, county, type of comm./admission (code #), doctor, sex, race, age, number of admissions for month & year, religion, ward, disposition.
Written in green ink in the log book are patients who were in the holding clinic or boarders/temporary.
At the end of each month there is a tally of admissions for that month. Also a tally for Family Care patients transferred in & where transferred from. There was also a tally for the month for patients in the holding clinic or temporary admissions, as it was later called.
On page 24 there is a list of patients who were in day care.
In Sept.1974 (page 55) boarder was changed to temporary & a column for status was added.Collection
