Date, case # (for first admission, re-admission, transferred in ), name, status, county, type of comm./admission (code #), doctor, sex, race, age, number of admissions for month & year, religion, ward, disposition (in later years the disposition was rarely filled in).
At the end of most months there is a list of Family Care patients transferred in & where they are transferred from. Sometimes there are several months of Family Care patients listed on one page.
Also at the end of each month there is a tally of admissions for that month.
