Date, new admission number or where first admitted, re-admission number, re-admission # for the month, re-admission # for the month transferred, name of patient, first adm., re-adm. other, type of commitment, county, co., private, physician, sex, race, religion, age at admission, social status, nativity, # of adm. for month, # of adm. for year, picture, mental diagnostic code #, physical diagnosis, when staffed, disposition (discharged, expired).
"picture" was omitted from admission information starting in June 1967.
In March of 1968 "physical diagnosis" & "mental diagnosis" was omitted & "ward" was added.
On the last page of the log is a record of some Day Care patients.Collection
