Date, first admission #, re-admission # from Missouri, re-admission # for others, transfer in, name of patient, county, type of commitment, doctor assigned, male, female, white, colored, age, # of admission for the month, # of admission for the year, religion, ward, when staffed, disposition (discharged, died).
In November 1968 race & when staffed was omitted.
Race was added back in June of 1969.
Whether staffed was added after name of patient in September 1969.
