Taped inside front cover are two pages entitled Senile Custodial Care Cases. This includes:
Patients name, admission date, county, age, classification.
Deaths information:
Date, new admission #, re-admission #, name of patient, county, co., private, ward physician, sex, race, religion, age at admission, date of admission, age at death, total time in hospital (yrs, months, days), death # for the month & year, mental diagnosis, cause of death, burial place, autopsy.
In February 1962 death code # was added after total time in hospital.
In July 1962 death code # was removed & the diagnosis was given a code # instead of writing out the diagnosis. Also ICD code was added after the cause of death. Anatomical was added after autopsy.Collection
